Not all language is literal, sometimes language is figurative.

You may read, they have 'butterflies in their stomach'

It's important to know that they don't have actual butterflies in their stomach.

The person is actually feeling nervous

You can't literally eat a horse, but you are so hungry you could feel like you could eat a horse.
