Each character must be filled with a specific value from its corresponding table

Why do I say the PCS index is not faithful?

There are 31 Root Operations

There are nine categories they fall into:

ROOT OPERATIONS THAT INVOLVE:

1. taking out all or some of a body part

2. taking out solids, liquids, or gases from a body part

3. cutting or separation only

4. altering the diameter or route of a tubular body part

5. putting in, putting back, or moving some or all of a body part

6. always involve a device

7. examination only

8. defining other repairs

9. defining other objectives
