Helps patients regain physical strength and movement following injury or surgery.

Used to restore patient to highest level of functioning possible after an accident, illness, or injury

The CNA is a vital team member, helping patients relearn to care for themselves and become active again.

Initiated once the goals of rehabilitation have been met or when rehab is not an option

Used to help ensure resident retains the skills learned in rehabilitation

Maintain a person's basic functioning, improve quality of life, increase independence

Examples: toileting, range of motion, grooming, eating/drinking, communication

The resident and their families

Understand diagnosis and disability

Be aware of resident’s limitations

Know resident’s abilities and strengths

Follow person-centered care plan

Maintain a positive, empathetic, and optimistic attitude

Encourage independence and personal choice as often as possible

Spend as much time as possible just listening to the resident

Offer praise whenever possible, and recognize that people will have setbacks in their rehabilitation, at that is okay!

Be observant! CNAs might see changes in a resident long before the nurse does.
